Troop 645 must register all scouts and re-charter the Troop each year.

Medical Form A recent copy or new Scout Personal Health and Medical Record and Permission Form for each scout. Class 1 is a minimum requirement
Automobile Insurance Declaration form Please include all adult drivers and vehicles that may be used for a troop activity.

Merit Badge Counselor Application
Please consider being a counselor for one of the many merit badges offered to scouts by
completing the enclosed application. A list of merit badges is on the back of the application. Denver Area Council has requested
us to register merit badge counselors annually. If you are already a merit badge counselor and want to continue with the same
badges you still need to register annually.
